The forms available are PDF files that can be read by Acrobat Reader. If you do not have acrobat reader you can download it free by clicking (http://www.adobe.com) Whenever possible forms have been made fillable. This means they can be filled out on your computer then printed out and sent to the Credit Union by mail or faxed to (541) 341-6432. For your information and possible concerns about confidentiality, the forms are filled out on your computer screen and the information can only be printed out. They cannot be saved to disk, nor can the information be sent electronically.
